Request Form

To request a group reservation, please complete and submit the following form. Upon receipt of your group
reservation request, Science Downtown will provide you with your groups confirmation number and an available
date and time. Science Downtown will make every effort to accommodate your first preferred date and time,
however reservations are issued on a first come first serve basis.

Educators: Included with your confirmation number you will receive a field trip planning guide package.
